Good Faith Estimate Notice
Global Chiropractic Plano Frisco
You have the right to receive a Good Faith Estimate explaining the expected cost of your medical care.
Under federal law, healthcare providers are required to provide a Good Faith Estimate to patients who do not have insurance or who choose not to use insurance for scheduled, non emergency services.
When You Will Receive a Good Faith Estimate
Global Chiropractic Plano Frisco provides Good Faith Estimates before services are rendered.
For services scheduled in advance, a written estimate will be provided within the timeframes required by law.
For same day or next day appointments, a Good Faith Estimate will be provided on the day of service prior to treatment.
You may request a Good Faith Estimate at any time by contacting our office.
What the Estimate Includes
A Good Faith Estimate reflects the expected charges for chiropractic services based on the information available at the time the estimate is prepared. Because chiropractic care is individualized, actual charges may vary depending on clinical findings, response to care, and treatment needs.
Billing Protections
If you receive a bill that is $400 or more higher than your Good Faith Estimate, you have the right to dispute the bill through the federal patient provider dispute resolution process.
We encourage you to retain a copy of your Good Faith Estimate for your records.
